What This Study Did, in Plain Language

What Was Tested

This registered phase 4 trial tested Tirzepatide.

This study is to investigate how adipocyte (fat cell) function and fat distribution differ between individuals with normal-weight type 2 diabetes (NWD), those with overweight type 2 diabetes (OWD), and normal-weight controls without diabetes (NWC).

It had three arms (groups): Normal weight controls (other), Normal weight with diabetes tirzepatide first pioglitazone second (experimental) and Normal weight with diabetes pioglitazone first, tirzepatide second (experimental).

Control.

Baseline testing Intervention with tirzepatide for 16 weeks Retesting Washout for 5 weeks Intervention with pioglitazone for 16 weeks Retesting.

The registry classes the interventions as drug, procedure and radiation.

More on the Plan

The investigators aim to learn how adipocyte dysfunction and disordered fat distribution contribute to the development of type 2 diabetes in normal-weight individuals.

This study is important because most diabetes research focuses on overweight or obese individuals, and very little is known about how diabetes affects those with normal body weight.

Who Took Part

104 planned participants were the target.

It was open to people of any sex aged from 30 years up to 70 years.

Healthy volunteers could join, not only people with the condition.

Examples of who could join: Age: 30 to 70 years old. BMI: Between 19 and 24.9 kg/m². Stable Body Weight: Body weight must have been stable (no more than 2 kg change) over the last 3 months. Age: 30 to 70 years old. BMI: Between 19 and 24.9 kg/m².

The full record lists 10 inclusion and 13 exclusion criteria; among the exclusions: pregnancy or lactation: women who are pregnant, planning to become pregnant, or breastfeeding are excluded due to potential risks to the fetus or infant; prior use of pioglitazone: participants who have previously used pioglitazone are excluded to avoid confounding effects of prior drug exposure; congestive heart failure or fluid overload history: these conditions are exclusionary due to the risk of fluid retention with pioglitazone.

How the Study Was Run

The set-up was randomized, crossover and open-label.

Because people in this Tirzepatide study were split into groups at random, differences between the groups are less likely to reflect who happened to join each one.

In the crossover set-up each of the participants took both Tirzepatide and the comparison in turn, acting as their own control.

Everyone knew when Tirzepatide was being given (open label), which is simpler to run but leaves more room for expectations to affect what is reported.

It started in 2025-01-13 with completion listed for 2027-12-15.

It ran at 2 sites in United States (2).

Its stated primary purpose is treatment.

What Was Measured

Researchers chose this as the main measure: Insulin resistance in normal weight women with diabetes compared to those with no diabetes (time frame: Baseline measures comparison between the groups). Adipocyte function in women with normal weight diabetes as compared with those without diabetes (time frame: Baseline comparisons). Fat storage in women with normal weight diabetes compared to those without diabetes (time frame: Baseline comparisons between the groups).

It also tracked 6 secondary outcomes, for example Effect of adipose-directed therapy pioglitazone on HbA1c in both men and women with normal weight diabetes, Effect of pioglitazone in insulin resistance in both men and women, Fat storage in men with normal weight diabetes (NWD) compared to those without diabetes (NWC), Adipocyte function in men with normal weight diabetes as compared with those without diabetes, Insulin resistance in normal weight men with diabetes compared to those with no diabetes and Insulin secretion in men and women with normal weight diabetes compared to those without diabetes.

What This Means in Plain Language

Put simply, this is a phase 4, randomized trial of Tirzepatide in 104 people.

It is still recruiting; no results are posted to the registry yet, so nothing here describes outcomes.

Our Notes on the Compound

Tirzepatide: A prescription peptide medicine that acts on both the GIP and GLP-1 receptors; Mounjaro and Zepbound contain the same active ingredient.
