What This Study Did, in Plain Language
What Was Tested
The trial (phase 4) is built around Semaglutide.
Roughly one patient in five never reaches the expected weight loss after MBS, and long-term regain after SG is well documented.
These patients are identifiable at the 12-month visit.
Today they are watched, coached, and treated only when failure is established.
It had two arms (groups): Immediate (active comparator) and Triggered (placebo comparator).
The registry classes the interventions as drug.
More on the Plan
Both strategies have a real case, which is what makes this a trial and not a formality.
For immediate treatment: the BARI-STEP effect size, the metabolic cost of two lost years, and the risk that established regain is harder to reverse.
Genuine equipoise, between strategies rather than between drug and nothing.
The design removes the ethical problem.
Who Took Part
Enrollment: 100 planned participants were the target.
It was open to people of any sex aged from 18 years up to 65 years.
Healthy volunteers were not accepted; participants needed the condition being studied.
Examples of who could join: Primary laparoscopic SG or RYGB at their unit or a participating unit. Able to self-inject and consent.
The full record lists 4 inclusion and 8 exclusion criteria; among the exclusions: any glp-1-based therapy since surgery; anatomical abnormality explaining the poor response (these patients go to the revision pathway, not this trial); type 1 diabetes.
How the Study Was Run
The set-up was randomized, parallel-group and quadruple-blind.
Random assignment meant chance, not the researchers, decided who got Semaglutide and who got the comparison, which limits bias.
Groups ran in parallel: each of the participants stayed in one arm throughout.
As a double-blind Semaglutide study, the assignments were hidden from both participants and investigators until the end.
Those kept unaware of assignments: participant, care provider, investigator and outcomes assessor.
It started in 2026-10 with completion listed for 2028-10.
It ran at 1 site in Egypt.
Its stated primary purpose is treatment.
What Was Measured
Primary outcome: Percentage total weight loss (time frame: at 12 months).
It also tracked 6 secondary outcomes, for example Percentage total weight loss, change in HbA1c, systolic and diastolic blood pressure, total cholesterol, LDL, HDL, and triglycerides, Hand-grip strength and change in IWQOL-Lite-CT total and domain scores.
What This Means in Plain Language
Put simply, this is a phase 4, randomized trial of Semaglutide in 100 people.
Its status is not yet recruiting; no results are posted to the registry yet, so nothing here describes outcomes.

Our Notes on the Compound
Semaglutide: The active ingredient in several prescription GLP-1 receptor agonist medicines (Ozempic, Wegovy, oral tablets).
